A caregiver-reported assessment covering 10 clinical domains — behavior, OCD, anxiety, food intake, tics, cognition/development, sensory, other symptoms, sleep, and health. Approximately 101 questions plus a short 5-item caregiver-wellbeing block. Your responses are delivered securely to your child’s care team.
We’ll ask a few short questions about you and your child, your child’s current flare status, and whether this is a first (baseline) or a follow-up assessment. Then the 10 clinical sections. Save and return any time.
You can stop after any question. A resume link is emailed to you and expires in 30 days.
Reports are delivered privately to your child’s care team. Nothing is posted publicly.
Some sections describe serious symptoms. You may skip any question — skipped questions are excluded from scoring.
